Company Description Treasure Coast Hospice is a non-profit community organization that provides compassionate, expert hospice and grief support services to patients and families at the end of life. Founded in 1982, it now serves more than 4,000 patients annually across Martin, St. Lucie, and Okeechobee counties. Role Description The Social Worker is a full-time, on-site role based in Stuart, FL, providing psychosocial support to hospice patients, caregivers, and families. Day-to-day responsibilities include conducting psychosocial assessments, developing care plans, facilitating individual and family counseling, and coordinating community resources to address emotional, social, and practical needs. The Social Worker collaborates closely with an interdisciplinary team including nurses, physicians, chaplains, and volunteers to ensure holistic, patient- and family-centered care. Additional duties include crisis intervention, advocacy, participation in bereavement programs, documentation in accordance with regulatory and organizational standards, and support for educational and outreach activities. The role requires consistent, compassionate communication and the ability to work effectively in home, inpatient, and community settings. Qualifications Professional social work skills, including psychosocial assessment, care planning, counseling, and crisis intervention. Strong communication, active listening, and interpersonal skills for working with patients, families, caregivers, and interdisciplinary teams. Organizational and documentation skills, including accurate record-keeping, time management, and attention to regulatory and ethical standards. Ability to collaborate in a multidisciplinary healthcare environment and coordinate with community agencies and support services. Demonstrated cultural sensitivity, trauma-informed practice, and commitment to person-centered, non-discriminatory care. Master’s or Bachelor’s degree in Social Work from an accredited program; state licensure or eligibility as required by Florida regulations. Experience in hospice, palliative care, medical social work, or related settings preferred. Valid driver’s license, reliable transportation, and ability to travel locally to patient homes and facilities as needed.
