Key Responsibilities: Provides primary health care to patients, according to established evidence-based guidelines of clinical practice. Ability to lead care teams to provide high quality healthcare in accordance with Horizon Quality Improvement Plan and meet established goals and benchmarks. Management of long-term care for acute and chronically ill patients, including care coordination and development of care plans and self-management plans. Successful candidates must be licensed or be able to successfully obtain licensure in the State of South Dakota.
