You will conduct evaluations, develop individualized treatment plans, deliver hands‑on therapy, and collaborate with our interdisciplinary care team — all while making a real difference in patients daily lives. Key Responsibilities Conduct comprehensive PT evaluations and develop individualized treatment plans Deliver skilled PT interventions including therapeutic exercise, manual therapy, gait training, and fall prevention Educate patients and caregivers on home exercise programs and safe mobility Complete accurate, timely clinical documentation per Medicare/Medicaid standards Collaborate with physicians, nurses, OTs, and other care team members Participate in care conferences and discharge planning as needed Qualifications Active, unrestricted Physical Therapist (PT) license in good standing Graduate of an accredited PT program (DPT or MPT) Minimum 1 year of PT experience (home health experience a strong plus) Valid driver license and reliable personal transportation Strong clinical documentation and communication skills CPR/BLS certified (or willing to obtain prior to start) Preferred Experience Home health or geriatric PT background OASIS certification or experience with OASIS-E assessments Experience with orthopedic, neurological, or post‑surgical populations Compensation Perks $90–$120 per visit (based on visit type and experience) Additional compensation available for evaluations and specialized visits Mileage reimbursement between patient visits Flexible PRN schedule — work as much or as little as youd like #J-18808-Ljbffr
