Description: Engage clients in community and office visits at least once weekly. Provide ongoing wellness and medication management, assessment, and other supports as needed. Deliver direct services related to the treatment and recovery of individuals living with mental illness according to guidelines. Demonstrate competencies in harm reduction, trauma-informed care, stage-wise treatment, motivational interviewing, and other evidence-based practices. Display core competencies in designated areas of practice, including core operating principles, integrated physical health and behavioral health, vocational services, family psychoeducation, and wellness self-management. Coordinate treatment and services with a variety of other teams and systems including inpatient units, primary and specialty care physicians, supportive housing programs, substance use rehabilitation programs, and respite care. Complete goal-oriented treatment planning such as plans of care, psychosocial assessments, and safety plans. Candidates must be comfortable conducting community visits with patients in various locations. Manage a caseload of approximately 13 to 18 patients.
