- Conduct evaluations for patients in inpatient or outpatient environments and develop personalized treatment plans
- Implement physical therapy interventions emphasizing mobility, strength, balance, and day-to-day functionality
- Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
- Provide education to patients and their families regarding therapeutic goals, discharge plans, and exercises to continue at home
- Maintain detailed records of evaluations, treatments, and patient progress compliant with regulatory standards
- Engage in care team discussions and interdisciplinary meetings to ensure seamless patient care
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
- Active state licensure as a Physical Therapist
- CPR certification strongly encouraged
- Specialized certifications in areas like geriatrics or neurology are advantageous but not mandatory
- Prior clinical experience is highly beneficial
- Ability to provide care for patients with complex medical histories and mobility challenges
- Strong communication abilities and teamwork orientation
