Position Overview


Seeking a Primary Care Nurse Practitioner (NP) to provide comprehensive, community-based healthcare services to seniors through a Program of All-Inclusive Care for the Elderly (PACE).


Under the direction of the Medical Director, the provider will collaborate with physicians and an interdisciplinary team (IDT) to conduct patient assessments, develop individualized care plans, coordinate healthcare services, and educate patients and their families.


The position emphasizes holistic, value-based care, smaller patient caseloads, and long-term relationships that support seniors in remaining in their homes and communities.
