Position Highlights:
- Physician-led, team-based care model
- Flexible schedule (4 or 4.5 days/week available)
- Focus on quality and outcomes, not volume

Required Qualifications:
- Unrestricted medical licensure in the state of Indiana or able to obtain prior to start
- Board certified or board eligible in Family Medicine or Internal Medicine or Med-Peds
- Active and unrestricted DEA License or ability to obtain prior to start
