The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

Senior Care Partners PACE is seeking a compassionate and innovative Occupational Therapist (OT) to join our interdisciplinary team and help participants live as independently and safely as possible in their homes and communities. This is more than a traditional therapy role. You'll have the opportunity to look at the whole person and their environment —helping participants overcome barriers, adapt to changing abilities, and continue doing the activities that matter most to them. Why This Role Matters
Your work will directly support participants in maintaining or improving their ability to perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs) —from personal care and mobility to household tasks and community living. You'll combine clinical expertise, creativity, education, adaptive strategies, and environmental modifications to help each participant achieve their individual goals and remain engaged in the community.
What You'll Do
- Assess & Develop Personalized Care Plans
- Conduct comprehensive initial and periodic occupational therapy assessments in participants' homes.
- Identify functional, cognitive, environmental, and safety needs.
- Develop participant-centered plans of care addressing home care and functional needs.
- Continually evaluate the need for occupational therapy and home care services and communicate recommendations to the Interdisciplinary Team (IDT).
- Provide Meaningful Therapy
- Plan and implement individualized treatment programs designed to improve or maintain ADL and IADL function.
- Provide therapeutic activities, exercises, compensatory strategies, and functional training.
- Provide training related to adaptive equipment and durable medical equipment (DME).
- Recommend adaptive equipment and environmental modifications based on medical necessity and participant goals.
- Help participants maximize independence, safety, confidence, and quality of life.
- Educate & Empower
- Educate participants, families, caregivers, and facility staff on functional and cognitive abilities, safety, adaptive techniques, and strategies that support independence.
- Provide practical recommendations that caregivers can incorporate into daily routines.
- Support participants and caregivers in achieving meaningful, individualized goals.
- Collaborate as Part of an Interdisciplinary Team
- Actively participate in participant-centered care planning and IDT meetings.
- Communicate changes in participant condition and functional status to appropriate team members.
- Participate in transitions from hospitals, assisted living communities, and skilled nursing facilities and provide recommendations to the IDT.
- Collaborate with Recreational Therapy on wellness programming and group activities.
- Support the development of innovative programs that benefit participants and align with organizational goals, vision, and values.
- Lead & Mentor
- Provide professional supervision and support to Certified Occupational Therapy Assistants (COTAs) and other staff implementing treatment programs.
- Provide ongoing education and guidance to COTAs.
- Ensure COTA supervision and practice comply with Senior Care Partners PACE policies and applicable Michigan occupational therapy regulations.
- Maintain Excellence in Care
- Maintain accurate and timely documentation in the participant's medical record.
- Participate in Quality Improvement initiatives.
- Follow organizational policies, professional standards, and applicable laws and regulations.
- Promote a patient- and family-centered approach in every aspect of care.
What We're Looking For
Required Qualifications:
- Doctoral degree from an accredited Occupational Therapy program, unless grandfathered under previous Occupational Therapy degree.
- Graduate of an Occupational Therapy program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE®) or an equivalent accrediting organization.
- Current, active, unencumbered license to practice as an Occupational Therapist in the State of Michigan.
- At least one year of experience working with frail, elderly, or long-term care populations.
- Demonstrated ability to collaborate effectively with members of an interdisciplinary care team.
- Strong communication, clinical reasoning, organization, and problem-solving skills.
- Commitment to participant-centered, compassionate care.

Minimum Working Requirements
- Must have reliable transportation, current valid Michigan driver's license and car insurance.
- Must be 18 years of age or older.
- Must be fully vaccinated, or willing to be fully vaccinated against the following communicable diseases: Influenza, MMR, Varicella, Tdap/Td, Covid-19 and Hepatitis B.
