Job DescriptionJob DescriptionCore Health Provider Services, Inc has an employment opportunity as a Direct Support Professional at our residential group homes. We are looking for individuals who have a sincere desire to help others. Our mission is to serve adults with special needs, by providing specialized services including assisting with ADL's, tracking goals, and community outings. Supports are individually planned and tailored to meet the specific needs of the individuals we serve. This may include working at multiple homes, locations, and departments.Position Details:PLEASE READ JOB DESCRIPTION AND RESPONSIBILITIES BEFORE APPLYING:ESSENTIAL DUTIES AND RESPONSIBILITIESAdheres to assigned schedules and reports to duty for the hours assigned and implements planned schedules and activitiesSupports in activities and companionship services for daily living, such as cooking, bathing, dressing, grooming, feeding, toileting, transferring, and other similar tasksAccompanies and transports to participate in visits for recreation, medical and dental care appointments, therapies, personal shopping and other community activitiesImplement ISP goalsAssists in creating a physical environment that lends to progress including house cleaning and laundryOther duties as assignedRequired Qualifications :The successful candidate must:Be motivated and self-directedHave a high school diploma or GED (proof will be required)Have dependable transportation, valid driver’s license, and valid car insuranceBe able to satisfactorily pass a criminal background checkProvide a clean 7 year driving record, with no suspensions due to moving violations in past 3 yearsMust obtain a TB testCurrent CPR/First Aid certificationCurrent physicalProven experience as personal care assistantWell-organized and reliableExcellent communication and people skillsPositive and compassionate attitudeCommitted to health & safetyOther Requirements: MUST complete required training education certification to continue professional development.Education:High school / GED (Required)
