Compassionate Hospice RN Case Manager

Visiting Nurse Association of Wisconsin, Inc. is seeking a compassionate Registered Nurse to coordinate end-of-life care for terminally ill patients and families across hospice settings. You will lead the care plan, educate patients and families, and ensure timely communication with physicians and the IDG. The role requires 1–2 years in nursing with home health, hospice, medical/surgical or oncology experience; Wisconsin RN license and BLS; strong assessment, documentation, and teamwork. #J-18808-Ljbffr
