About the Position

Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum of 1 year current acute hospital experience. Must have 2 years of acute care hospital inpatient case manager experience. License: Must hold a valid state license. Certifications: Required certifications for case management. Must-Have: Knowledge of Interqual and Epic systems. Ability to assess patients, coordinate care, and develop discharge plans. Experience with care coordination and admission reviews. Must commit to working at least 2 weekends in a 4-week schedule. Must float between facilities within a given state. Description: The registered nurse will operate as a case manager with a patient ratio of 1:20. The daily caseload will be up to 20-22 patients. Responsibilities include assessing patients, coordinating care, and planning for patient discharge. Experience with admission criteria is required. Ability to utilize Interqual criteria for care coordination is essential. Candidate must have current acute hospital experience and be able to work holidays as required. Must provide two references from within the last three years. Candidates should be prepared to comply with health-related policies. Floating between facilities is expected, and refusal to float may lead to termination of the assignment. Communication devices may be provided as needed for the role.
