Nurse Practitioner or Physician Assistant - Outpatient Primary Care - Girard

Company : Allegheny Health Network Job Description : GENERAL OVERVIEW: In addition to your base compensation, this role offers an annual quality-based bonus. This bonus is directly tied to contributions to population health initiatives and value-based metrics, rewarding teamwork, improving patient outcomes and driving high-quality care. This job is an integral part of the care delivery system, providing primary healthcare services for non-acute or chronic health conditions in specified populations in accordance with applicable scope and standards of practice and with the organization’s policies, values, and mission within the CRNP scope of practice. Work is performed in a physician office or offices dedicated to Primary Care, with weekend and on-call coverage on a rotational basis as required. ESSENTIAL RESPONSIBILITIES Provides appropriate patient care including recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, and documenting medical records. Communicates and coordinates patient care with patient, family, community health services, and the interdisciplinary team. Involves the attending physician regarding plan of care and physician in clinical decision making for issues outside the CRNP’s knowledge base or scope of practice. Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the CRNP and supervising physician(s). Prepares written prescription orders for drugs and controlled substances, administers medications and injections, sutures wounds, and provides wound care. Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions, and recommends community resources as appropriate. Triages patient telephone calls and provides monitoring and continuity of care. Participates in, develops, and implements performance and/or quality improvement initiatives. Ensures compliance with all regulatory agencies. May perform other duties such as managing outpatient concerns and any other duty specific to the specialty. Other duties as assigned or requested. QUALIFICATIONS: Required Master's degree MSN from an approved state board of nursing master’s program National Board Certification for Nurse Practitioners through the AANP, ANCC, NCC, or PNCB Practicing-state licensure as a Nurse Practitioner Prescriptive Authority from the practicing state within 60 days of hire Drug Enforcement Agency Licensure within 60 days of hire CPR – American Heart Association Act 34 Criminal Background Clearance Certificate Act 33 Child Abuse Clearance Certificate Act 73 FBI Fingerprinting Criminal Background Clearance Certificate Preferred None Required Master’s degree in Physician Assistant Studies Current PA Physician Assistant License PA-C or board eligible. Ongoing successful recertification (NCCPA) as a PA-C within 90 days of hire Drug Enforcement Agency Licensure within 60 days of hire CPR - American Heart Association Act 33 Child Abuse Clearance Certificate Act 34 Criminal Background Clearance Certificate Act 73 FBI Fingerprinting Criminal Background Clearance Certificate Substitutions Four to ten years related experience and/or training; or equivalent combination of education and experience.
