Schedule Monday - Friday, 8:00am - 4:30pm Weekend and On-call Rotation Perks Cell phone and laptop Generous mileage reimbursement at IRS rate Up to $15,000 sign‑on bonus Relocation available to eligible candidates Location St. Louis City/County Responsibilities Assess patient preferences and barriers to involvement in care, including their values, emotional, spiritual, cultural, and population‑specific needs. Develop, implement, and document individual plans of care with defined goals in collaboration with other members of the interprofessional team and patient, family or caregiver in accordance with the established guidelines and standards of nursing care. Proactively plan and ensure communication of the plan of care across the continuum of care. Promote respect, equity and empathy in interactions with diverse and vulnerable populations through care delivery (e.g., support for emotional, spiritual, and cultural preferences of patient, family and/or caregivers). Practice collaborative problem solving, service recovery and advocacy for patient family‑centered continuity of care. Implement care by integrating data from the interprofessional team and critical thinking in a safe and timely manner. Evaluate changes in patient’s condition, inform and collaborate with family and/or caregivers, and communicate with interprofessional team as changes occur in plan of care, update the plan in EMR. Evaluate current nursing care to ensure evidence‑based practice and quality patient outcomes. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.
