Physical Therapist (PT) – Tomah & Sparta, WI
Skilled Nursing Facility is seeking a highly motivated and energetic Physical Therapist (PT) to join the team serving Tomah and Sparta, WI. Candidates must be willing to support a friendly, positive, and professional environment and complete a 13-week travel assignment.
Position Highlights:
- 13-week travel contract
- Skilled Nursing Facility setting
- Split coverage between Tomah and Sparta
- Full-time opportunity

Requirements:
- Active Wisconsin Physical Therapist license (or eligibility)
- Previous SNF, LTC, or geriatric experience preferred
- Strong communication and clinical skills
- Ability to work independently and collaboratively
