Wound Care Nurse Practitioner / Physician Assistant (Home Health)
Location: Raleigh, Charlotte, NC
Job Type: Full-time
Shifts: Days (Monday-Friday)
Summary:
We are looking for a skilled Nurse Practitioner (NP) or Physician Assistant (PA-C) to provide advanced wound care services to patients across the Raleigh area. This is an excellent opportunity for clinicians who value autonomy and patient-centered care in a home-based setting.
Responsibilities:
- Conduct detailed wound assessments and follow treatment plans
- Track and document healing progress using best-practice protocols
- Provide education to patients and caregivers on wound care techniques
- Coordinate with physicians and interdisciplinary care teams
- Ensure accurate and timely EHR documentation
- Travel to patient homes within Raleigh and surrounding areas
Qualifications:
- Active NP or PA-C license in North Carolina
- Wound care experience is a plus
- New graduates are welcome to apply with relevant clinical exposure or hands-on experience in wound care settings
- Ability to work independently in a field-based role
- Valid driver's license
- Comfortable working in a home care environment
