Bilingual Case Manager - In-Home Senior Care (Austin)

LifeSpan Home Health in Austin, TX is seeking a bilingual Case Manager to coordinate services across STAR+PLUS, STAR Kids, PHC, and Family Care. The role includes direct personal care when needed to ensure high-quality support for clients living at home. The ideal candidate will have 2+ years of supervisory experience, strong communication, and familiarity with PAS program management.
