Ambulatory Care Pharmacist - Primary Care

Hartford HealthCare Medical Group is a large medical practice in New England with multiple locations in Connecticut and Rhode Island. This role is based in our Connecticut clinics. Responsibilities The Ambulatory Care Pharmacy Specialist collaborates as an integral member of the multidisciplinary team in the provision of care for patients in our medical clinics to optimize and promote rational, safe, evidence-based and cost-effective therapy. Responsibilities include actively participating in committees and/or workgroups, collaborating in the review of new and existing medications, clinical practice guidelines and other related policies used in the management of clinic-specific medical conditions, and conducting clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication‑use processes or pharmacy practice. Education B.S. or Pharm.D. required. Pharm.D. preferred. Licenses/Certifications Current CT Pharmacist license or eligibility within 45 days Experience PGY‑1 Pharmacy residency preferred Excellent customer service and communications skills Prior experience with EPIC systems preferred. #J-18808-Ljbffr JobRx, Inc.
