In collaboration with a supervising Physical Therapist, you will execute individualized treatment plans to help patients improve strength, mobility, balance, and overall independence. This position calls for a compassionate, self-motivated professional who enjoys field-based work and excels at collaborating with an interdisciplinary team.Schedule:Full-time, 13-week contractMonday Friday, 8:00 AM 5:00 PMExpected caseload: 2532 points/week (approximately 57 patient visits per day)Daily travel within an assigned territory (mileage reimbursement provided)Key Responsibilities:Deliver skilled therapy interventions according to the established care plan developed by the PTGuide patients through therapeutic exercises, gait and balance training, transfer assistance, and functional tasksTrack and document patient progress and responses to treatment using Home Care Home Base (HCHB)Report changes in patient condition to the supervising PT and other care team membersReinforce patient and caregiver education on exercises, safety strategies, device usage, and fall preventionMaintain accurate, timely electronic documentationEngage in interdisciplinary communication and participate in team meetings and case conferencesUphold all policies, regulatory standards, and Medicare home health guidelinesQualifications:Active, unencumbered Physical Therapist Assistant license in state of practiceMinimum of 1 year of recent Home Health PTA experience preferredFamiliarity with HCHB and OASIS documentation processes preferredReliable transportation, valid drivers license, and auto insurance requiredExcellent organizational, time management, and communication skillsAbility to work independently in a flexible, field-based roleBenefits include weekly pay, options for medical, dental, and vision insurance, 401(k) retirement plans, 24/7 support, and mileage reimbursement, with potential for contract extensions.Advance your career delivering high-quality care where it matters most.
