Job Summary: Under minimal supervision and at the direction of the Case Management Director , the Inpatient Social Worker/Patient Counselor supports patients in addressing their bio-psychosocial needs, facilitating access to medical care and community resources to optimize their health outcomes. This role involves crisis intervention, short-term counseling , and transition planning , ensuring patients move smoothly along the continuum of care. The Social Worker/Patient Counselor also coordinates discharge planning by collaborating with patients, families, and healthcare teams.
Ideal Candidate Profile: The ideal candidate is a compassionate and resourceful professional with strong problem-solving skills. They demonstrate expertise in crisis intervention, patient counseling, and care coordination while maintaining a patient-centered approach .

- Conduct bio-psychosocial assessments to determine patient needs.
- Provide crisis intervention and short-term counseling for patients and families.
- Assist patients in accessing medical care and community resources .
- Develop and execute discharge plans , collaborating with healthcare teams.
- Support patients transitioning across different levels of care.
- Ensure compliance with healthcare regulations and ethical standards.
- Maintain accurate documentation of patient interactions and treatment plans.
- Master's Degree in Social Work or Counseling Psychology .
- Minimum one year of experience as a Social Worker or Counselor.
- Two years of experience in a hospital setting.
