This is an excellent opportunity for a therapist who enjoys working with medically complex patients and collaborating with physicians, nurses, occupational therapists, speech-language pathologists, and case management professionals to improve patient outcomes. What You'll Do Evaluate patients and identify functional limitations, mobility impairments, and rehabilitation needs. Develop individualized treatment plans designed to maximize mobility, safety, independence, and recovery. Deliver evidence-based interventions including therapeutic exercise, gait training, transfer training, balance activities, neuromuscular re-education, and patient education. Assess discharge needs and collaborate with the care team to support safe transitions to home, inpatient rehabilitation, skilled nursing facilities, or other levels of care. Educate patients and family members regarding treatment plans, home exercise programs, fall prevention, and recovery strategies. Document evaluations, treatment sessions, progress notes, and discharge recommendations in accordance with organizational and regulatory standards. Collaborate closely with interdisciplinary care teams to ensure coordinated, patient-centered care. Exercise independent clinical judgment while managing a diverse caseload of acute care patients. Support department initiatives, quality improvement efforts, and clinical education activities when appropriate. Patient Populations You may have the opportunity to work with a variety of patient populations, including: Medical-Surgical Orthopedic Pulmonary Critical Care/ICU Complex Medical Cases Qualifications Required Graduate of an accredited Physical Therapy program. Current New Jersey Physical Therapy License or eligibility to obtain licensure. Strong communication and interpersonal skills. Commitment to delivering exceptional patient-centered care. Preferred Previous experience in acute care, inpatient rehabilitation, or hospital-based physical therapy. Experience working within a large health system or academic medical center. Clinical affiliations or rotations completed in acute care settings. Board Certification or pursuit of specialty certification.
