About the Position

As a PRN RN with CompleteOK Home Services, you'll conduct nursing visits for clients receiving services through Oklahoma's ADvantage Waiver Program.

You'll play an important role in evaluating client needs, monitoring their well-being, and helping ensure they receive appropriate support to remain safely in their homes.

This position is ideal for an organized, self-motivated RN who enjoys working independently and building meaningful relationships with clients and families.

Service Area: Sequoyah County, Oklahoma, including Sallisaw and surrounding communities.

What You'll Do

• Conduct scheduled nursing visits for ADvantage Program clients in their homes.
• Complete nursing assessments and reassessments as assigned and required by program guidelines.
• Evaluate clients' health status, functional abilities, and ongoing care needs.
• Identify and document changes in client conditions and communicate concerns to the appropriate care team members.
• Provide client and caregiver education related to health, safety, and maintaining independence at home.
• Collaborate with case managers, caregivers, and other healthcare professionals to support coordinated care.
• Complete accurate and timely documentation in accordance with agency policies and applicable ADvantage Program requirements.
• Help ensure services are delivered in accordance with established care plans, program guidelines, and professional nursing standards.

What We're Looking For

Required Qualifications:

• Current, unrestricted Registered Nurse (RN) license in Oklahoma.
• Strong nursing assessment and clinical judgment skills.
• Excellent communication, documentation, and organizational abilities.
• Ability to work independently and manage assigned visits efficiently.
• Valid driver's license, reliable transportation, and current automobile insurance.
• Ability to travel throughout Sequoyah County and surrounding assigned service areas.
• A genuine commitment to compassionate, client-centered care.

Preferred Qualifications:

• Previous experience with Oklahoma's ADvantage Waiver Program.
• Experience in home health, home and community-based services, case management, or geriatric nursing.
• Familiarity with nursing assessments, care coordination, and electronic documentation.
