Job Opportunity: Licensed Practical Nurse

Our organization is in search of a committed Licensed Practical Nurse to become a part of our Healthcare & Medical Services team at the Health Center in Vermilion, OH. In this position, you will deliver exceptional, resident-focused nursing care within an assisted living, rehabilitation, and memory care setting. Under the guidance of registered nurses and physicians, your responsibilities will include administering medications and treatments, conducting clinical assessments, accurately documenting care, and assisting in the creation and implementation of personalized care plans that enhance residents’ physical, emotional, and social well-being.

Key Responsibilities

• Monitor vital signs and identify changes in residents' conditions, reporting them promptly.
• Collaborate with interdisciplinary team members to ensure timely interventions.
• Assist with wound care, injections, catheter care, and other skilled nursing tasks within your scope.
• Provide education and emotional support to residents and their families regarding treatment plans and care objectives.
• Promote safety and infection control while adhering to all state and federal regulations, as well as facility policies.
• Supervise and guide nursing assistants and other care team members to uphold high care standards.
• Engage in effective communication with physicians, therapists, social workers, and other healthcare professionals to coordinate services.
• Participate in care conferences, maintain electronic health records, and contribute to quality improvement initiatives aimed at enhancing resident outcomes and satisfaction.

Qualifications

• Must be a licensed practical nurse in the state of Ohio.
