POSITION
Social Services Advocate

DESCRIPTION
Investigate and resolve complaints made by or on behalf of residents living in long-term care facilities. Provide advocacy and mediation to ensure resident rights and quality of life are maintained.
Key Duties
  • Conduct regular, unannounced visits to residents in long-term care facilities
  • Identify, investigate, and resolve resident complaints
  • Provide education, advocacy, and mediation on resident rights
  • Maintain professional relationships with nursing and board & care facility staff
  • Document case management activities accurately and in a timely manner
REQUIREMENTS
  • Bachelor's degree in social work, social services, gerontology, or related field - OR three or more years of related experience in LTC advocacy, social services, case management, or conflict resolution
  • Certified as a State Long-Term Care Ombudsman or the ability to successfully complete the 36- hour State Certified Long Term Care Ombudsman training.
  • Strong communication, problem-solving, and case documentation skills
  • Valid CA driver's license, auto insurance, and personal vehicle; ability to travel 60% within Riverside County
  • Bilingual Spanish a plus
  • Must pass LiveScan background check and provide proof of negative TB status
ADDITIONAL INFORMATION
Candidates must not have worked in a skilled nursing facility, assisted living facility, or board and care facility within the past 12 months.
