Hospice Intake Specialist

Full-Time | Monday–Friday | 8:00 AM–5:00 PM In-Office Position

Dependable Hospice is seeking an experienced and compassionate Hospice Intake Specialist to join our team. This position plays an important role in helping patients and families transition smoothly into hospice care while ensuring referrals and admissions are handled accurately, efficiently, and with compassion.

The ideal candidate is organized, responsive, and comfortable working in a fast-paced healthcare environment while coordinating with families, referral sources, clinical staff, and insurance providers.

• Receive and process new hospice referrals from hospitals, physicians, facilities, families, and other referral sources

• Gather required patient information and documentation to support timely admissions

• Communicate with patients and families regarding hospice services and the admission process

• Coordinate referrals and admissions with nurses, social workers, providers, and other members of the hospice team

• Verify insurance coverage, benefits, and eligibility for hospice services

• Accurately enter and maintain patient information in the EMR

• Track referrals and follow up on outstanding documentation or information

• Maintain communication with referral sources and provide timely status updates

• Complete intake and admission documentation accurately and efficiently

• Ensure intake processes comply with Medicare, Medicaid, state, and organizational requirements

• Protect patient confidentiality and follow all HIPAA requirements

• High school diploma or equivalent required

• 1–2 years of healthcare intake, hospice, home health, or related healthcare experience required

• Hospice intake or referral coordination experience strongly preferred

• Knowledge of insurance verification and Medicare/Medicaid guidelines preferred

• Experience working with EMR systems

• Strong organizational skills with the ability to manage multiple referrals and priorities

• Excellent written and verbal communication skills

• Compassionate and professional when working with patients and families

• Strong attention to detail and follow-through

Full-Time Monday–Friday | 8:00 AM–5:00 PM

Occasional after-hours support may be needed based on patient and organizational needs.
