Enjoy retention bonuses and career advancement opportunities in a supportive environment.Job DescriptionHospice Team Manager OverviewLead as a Hospice Team Manager, ensuring exceptional care delivery in a hybrid work environment.Collaborate with interdisciplinary teams to optimize patient outcomes and support systems.Provide expert guidance on pain management and symptom control strategies.Oversee compliance with healthcare regulations and documentation standards.Support onboarding and continuous professional development for hospice staff members.Participate in community outreach initiatives to promote hospice services and awareness.Adapt to flexible scheduling needs, including rotating weekends as required.Contribute to a culture of excellence and compassionate care within the organization.Hospice Team Manager Key Responsibilities & DutiesSupervise and mentor a team of nursing professionals to achieve excellence in hospice care.Coordinate staff assignments ensuring optimal patient-to-staff ratios and adherence to policies.Monitor and ensure quality and compliance of nursing documentation with regulations.Empower RN Case Managers to excel in patient care and symptom management.Participate in interdisciplinary team meetings to contribute to comprehensive care planning.Facilitate community presentations to enhance hospice service awareness.Support orientation and quarterly training sessions for staff development.Ensure adherence to organizational policies and procedures in daily operations.Hospice Team Manager Job RequirementsBachelor of Science in Nursing (BSN) from an accredited institution.Minimum of five years of hospice nursing experience required.Supervisory or leadership experience in a healthcare setting preferred.Active New York State RN licensure required.National Board Certification in Hospice and Palliative Care (CHPN) preferred or obtainable within one year.Proficiency in Word, Outlook, and Windows applications.Strong ability to supervise and mentor nursing staff effectively.Capability to work collaboratively within interdisciplinary teams.
