JOB REQUIREMENTS

• Board: Board Certified or Board Eligible in Urology
  
• Licenses: Active PA license or Universal License (IMLC) required
  
• DEA: Yes, with PA address
  
• Experience: Minimum 1 year; new graduates accepted
  

SUBMISSION REQUIREMENTS

• All required documentation must be submitted no later than 5 business days prior to start
  
• Provider questionnaire, NPDB self-query (within 6 months), and full malpractice disclosure required at presentation
  
• Submissions must include exact availability dates if specific coverage dates; no open-ended presentations
  
• Submissions without rates will be rejected
  
• Required submission packet: CV, coversheet, questionnaire, NPDB (within 6 months)
  
• Screening: malpractice settlement over $500K or more than 3 settlements (except cases >20 years) disqualifies
  
• Licensure: PA xywuqvp license or IMLC LOQ in hand; DEA with PA address required
  
• Credentialing: 90 days; temps available only for urgent, clean files; must be fully cleared 2 business days before start
  
• Work history: 7 years or 10 assignments
  
• Vaccination: proof of vaccination or positive titers for HepB and childhood vaccines; no declinations accepted
  

Duties & Responsibilities

• Provide 24-hour beeper call coverage, responding to calls within callback time
  
• Perform urological procedures: vasectomy, cystoscopy, prostate procedures, ureteroscopy, lithotripsy, orchiopexy
  
• Supervise nurse practitioners and physician assistants (new APPs)
  
• Conduct office visits and manage patient panel (24-28 patients daily)
  
• Utilize Epic EMR for documentation
