S.T.E.P.S. Family Service Specialist

Description

Plan and organize work to meet all job requirements according to agency and COA standards, licensing and funding requirements.
Responsible for working with the family to uncover youth/family strengths and needs, aids with immediate needs, and completion of the initial strengths assessment, needs, and priorities within (30) days of enrollment.
Ensures initiative-taking and reactive crisis/safety plans within 72 hours of learning about a potential crisis. Plans must reflect the best possible fit with strengths, culture and beliefs of the family.
Ensures collaboration with the child and family team to develop an individual plan of care (POC) with services and resources that are community based and culturally relevant within 90 days of enrollment.
Created updated plans of care whenever the team identifies changes that would aid in progress to reaching desired outcomes.
Ensure needs and services identified by the child and family team are met.
Ensure community resources are sought first with the assistance of the child and family team and community team.
Collaboration with individuals the youth and family has contact with, such as DHHS/CMO workers, teachers, judges, district attorneys, mental health clinicians, physicians, etc.
Ensures all barriers related to plan implementation are addressed.
Facilitates necessary referrals, information, and cooperation with collateral resources to ensure quality care and service delivery.
Monitor the provisions and quality of services provided to the family and is the liaison when new services and resources need to be sought or developed.
Complete all necessary paperwork in a strength-based manner per the agency requirements.
Develop and maintain liaison relationships with various community agencies, groups, volunteers, and key stakeholders.
Assist in advocacy for the development and provision of appropriate community resources and services for the youth and family.
Facilitate alternative placement of a youth if necessary. Participate in, contribute to staff case reviews and staff training.
Attend mandatory agency meetings and trainings.
Provides or secures support and crisis/emergency services for the youth/family. This may be done through face-to-face contact or phone contacts.
Ensure availability to testify in court if needed.
Provide transportation to appointments, crisis and respite services, etc. for youth, when necessary.
Engage families in wraparound process.
Prepare for and participate in development and coaching meetings with supervisor and attend team meetings as designed.
Represent the agency in the community and work in collaboration with others to secure needed resources.
Accountable for expenditures against program budget and work toward cost-effective operations.
Ensure monthly, quarterly and annual reporting is completed accurately and submitted in a timely manner.

Requirements

Required:

Bachelor's degree in human services field, from accredited school/university, in the areas of social work, psychology, sociology, criminal justice or guidance counseling.
2 years of experience working with youth in a residential or community based setting.
Access to personal vehicle required.
Valid driver's license without relevant restrictions.
Valid vehicle registration and insurance.

Preferred:

1-2 years of same or similar experience (i.e. independent living youth)
1-2 years of case management experience.
Child and Adolescent Functional Assessment scale (CAFAS) trained.
