Job Summary: The Early Start Service Coordinator at San Andreas Regional Center is responsible for coordinating and facilitating services for infants and toddlers with developmental delays and disabilities. This role involves working closely with families, service providers, and multidisciplinary teams to ensure that children receive the necessary early intervention services and support to reach their full potential. The Service Coordinator plays a key role in improving the lives of children and their families through assessment, planning, and ongoing support.
Essential Job Functions
- The Early Start Service Coordinator collaborates in psychosocial and developmental screening assessments in the home environment.
- The Early Start Service Coordinator must demonstrate a strong commitment to serving our community by being willing to travel extensively within assigned catchment areas that SARC covers.
- The Service Coordinator conducts vital home visits in diverse settings, with a particular emphasis on the child and family's natural environment. This approach fosters a more comprehensive understanding of the unique needs and challenges that each family faces, allowing us to tailor our support and interventions effectively.
- Develops Individualized Family Service Plans (IFSP) in collaboration with families and other professionals.
- Facilitate communication and coordination among various service providers, therapists, and specialists to ensure the child's overall development and progress.
- ES Service Coordinator will develop, implement, and coordinate an Individual Family Service Plan (IFSP) for each individual.
- Coordinate services for infants and their families; determining ongoing eligibility of clients at age 3.
- Completes all required forms, documentation, and reports according to regulations and SARC policies and procedures.
- Promote advocacy by parents and help parents become their children's teachers in everyday life skills, provide informational support to families, ensuring they are well-informed and actively engaged in the decision-making process regarding their child's care.
- Schedule and facilitate annual reviews, semi-annual reviews, transition planning conferences, clinical consultations, and ID/IFSP Team meetings as required.
- Works collaboratively with the service provider/community partners to develop and monitor a child's progress in his development and IFSP goals.
- Maintain accurate records of assessments, IFSPs, and ensuring compliance with regulatory standards.
- The ES Service Coordinator supports the child's transition from Early Start to educational services available after age 3.
- Provide advocacy, education, assist in crisis intervention, and other support for families, individuals, and care homes.
- Transfers, synthesizes, and reports individual statistical data as requested, including state surveys, internal surveys, and community agency surveys.
- Participate in internal and external committees for purposes including public education, improving internal agency functions, and promoting positive communication with external agencies.
- Participate in in-service and out-service training to increase knowledge and skills in the field of developmental services and other related areas.
- Stay up to date with current early intervention practices and regulations through continuous professional development and training.
- Serve as Officer of the Day (OD as assigned).
- Completes all required non-individual related documentation, including travel input, verification of time input and logs, monthly activity forms, and other forms as required.
- Gathers data and produces initial and/or ongoing assessments.
- Presents assessment data to Interdisciplinary Team (IDT) for eligibility determination.
- Facilitate the planning process and develop the Individualized Family Service Plan for assigned individuals and oversee the implementation of the IFSP. Monitor and evaluate the services provided through the plan.
- Advocates with individuals/families to assure provision of needed services.
- Initiates and follows up on referrals to existing community resources.
- Identifies areas of unmet needs.
- Performs tasks which relate to fiscal and accountability requirements.
- Provides assistance to existing and in the development of new community services.
- Completes all required caseload documentation.
- Develops expertise in accessing generic resources.
- Assures that all individuals and families are informed verbally and in writing of their right to appeal every time a decision is made to change, alter, or deny a service.
- Knows Part C Federal Regulations, State Regulation and Lanterman .
- Implement San Andreas Regional Centers policies and procedures.
- Collaborate effectively in crisis situations such as CPS, to eventually effect stability.
- Performs annual, semi-annual, Transition planning and quarterly visits as required.
- Complete all job functions in a timely manner.
- Demonstrates good over-all organization of work.
- Strong Oral and written communication skills.
- Has basic understanding of clinical issues and child development.
- Perform other duties as assigned.
- Keep District Manager/Supervisor informed of the status of the caseload and any issues that should arise.
- Collaborates with the family in obtaining essential diagnosis and evaluation from appropriate practitioners.
- To be a team player and work well with all providers in the community.

Requirements
Education/Experience: A Bachelor’s Degree in Social Work, Rehabilitation Counseling, or a related field from an accredited college, or a Bachelor’s Degree in Social Work plus four years of relevant experience, is required. The specific field of study for the Bachelor's Degree and the post-Bachelor's years of experience may be waived by the Executive Director if necessary to meet a special need of the agency, such as providing services to specific cultural or monolingual groups.
Special Skills and Knowledge: Knowledge of principles, objectives and techniques of social work; individual and group behavior, human growth, development of family relationships; medical-psycho-socio-economic factors affecting individuals and families; techniques of intervention, referral and follow-up to community resources and facilities and various community organizations, and how to intervene constructively on the client's behalf. Ability to relate in a constructive and enabling manner to a wide variety of individuals and organizations; ability to relate in a positive manner with supervisor, co-workers, and others in the agency and community; establish and maintain rapport with clients and their families; prepare and present oral and written reports; maintain case records in a clear, concise, appropriate and timely manner.
Other: Use of an automobile, possession of a valid driver's license and automobile liability insurance for the minimum amount prescribed by law, OR ability to provide for independent transportation.
