Nurse Practitioner

Position Overview Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations and other services in members’ homes, helping to simplify health care on a community-wide basis. Responsibilities Conduct In-Home evaluations lasting about 45 minutes, including a medication and medical history review, a physical evaluation, and optional diagnostic tests such as diabetic eye exams, spirometry, or bone density screening. Provide education and answer health questions during each visit. Fulfill the clinical duties specified for the role, including participating in diagnostic & preventive services products such as spirometry, DEE, bone density screening, etc. Travel locally or within licensed states and conduct virtual visits as needed. Work safely in diverse home environments that may contain pets, secondhand smoke, or allergens. Qualifications Active, unrestricted license(s) in the coverage area(s). Board certification (required). Multi‑state licenses (preferred). Ability to cover primary locations such as Concord and the surrounding area. Qualified as a Nurse Practitioner or Physician Assistant with required credentials. Competitive pay: $41.85 – $90.13 per hour, base rate depends on experience, education, geography and other factors.
