Now Hiring: Full-Time Registered Nurse (Rn) Field Nurse

Virginia Home Care Services is seeking a dependable, compassionate, and clinically competent Full-Time Registered Nurse (Rn) Field Nurse to provide skilled nursing services to patients in the comfort of their homes throughout Northern Virginia. We are looking for a nurse who is confident in clinical assessment, organized with documentation, and committed to delivering high-quality, patient-centered care.

Position Responsibilities

• Perform skilled nursing visits in patients' homes in accordance with physician orders and individualized plans of care.
• Complete Start of Care (SOC), resumption of care, recertification, routine skilled nursing, and discharge visits as assigned and qualified.
• Conduct comprehensive assessments and complete required OASIS documentation accurately and timely.
• Monitor patient conditions, identify changes in health status, and promptly communicate concerns to the appropriate clinical personnel and physicians.
• Perform medication reconciliation and provide patient and caregiver education.
• Manage chronic conditions, assess wounds, monitor treatment responses, and provide other skilled nursing interventions within the nurse's competency.
• Coordinate care with physicians, therapists, caregivers, and the interdisciplinary team.
• Maintain timely, complete, and compliant electronic medical record documentation.
• Promote patient safety, continuity of care, and prevention of avoidable hospitalizations.
• Follow Medicare requirements, Virginia regulations, and agency policies.

Qualifications

• Current, unrestricted Registered Nurse license in Virginia.
• Home health experience preferred; experienced nurses with strong clinical skills are encouraged to apply.
• OASIS and Medicare home health documentation experience preferred.
• Strong clinical assessment, critical-thinking, and time-management skills.
• Excellent communication and organizational abilities.
• Ability to work independently while collaborating with the clinical team.
• Valid driver's license, reliable transportation, and willingness to travel throughout the assigned territory.
• Ability to use an electronic medical record system and complete documentation within required timeframes.
