Job Overview

General Summary of Position
This role involves collaborating with healthcare providers to ensure the safe and effective use of specialty medications. The aim is to enhance communication within the patient's healthcare team by reviewing clinical guidelines, developing protocols, conducting medication reconciliations, creating patient care plans, and offering consultations. These efforts are designed to improve medication adherence, overall health, and patient knowledge. Additionally, the position requires documenting and reporting healthcare interventions and outcomes, as well as participating in the maintenance of specialty pharmacy accreditation.

Key Responsibilities

• Support the achievement of departmental goals and objectives while adhering to policies, procedures, quality, and safety standards. Ensure compliance with governmental and accreditation regulations.
• Verify medication orders, design dosage regimens, and provide effective administration and drug therapy monitoring services.
• Create, update, and maintain patient care plans for specialty medications.
• Educate patients about specialty medications and consult with healthcare providers.
• Gather patient and drug-related information for initial assessments, medication reconciliations, care planning, documentation, and follow-up.
• Oversee activities related to specialty pharmacy within the clinic environment.
• Document all services in accordance with specialty pharmacy accreditation standards and payer requirements.
• Engage in collaborative practice agreements with prescribers to enhance patient care.
• Assist in the implementation and administrative processes associated with new clinical programs and medications requiring specialized monitoring.
• Coordinate with relevant clinical and administrative personnel to improve operational efficiency and productivity.
• Stay updated with evidence-based clinical practice guidelines to ensure patient care plans and consultations reflect the latest research.
• Act as a preceptor for student pharmacists, summer interns, and residents when applicable.
• Contribute to the maintenance of specialty pharmacy accreditation.
• Participate in meetings and committees, representing the department and organization in community outreach initiatives.
• Engage in multidisciplinary quality and service improvement teams.

Minimum Qualifications Education

• Doctoral degree (PharmD) from an accredited college of pharmacy is required.

Experience

• A minimum of 3-4 years of direct patient care experience in a clinical setting, preferably within a specialty pharmacy.
• Experience in ambulatory care is preferred.

Licenses and Certifications

• Licensed Pharmacist (PHARMD) in Maryland or the District of Columbia, as applicable to the practice site, is required upon hire.
• Eligibility for reciprocity in surrounding states is required upon hire.
• Completion of an ASHP-accredited residency or other approved advanced training program is preferred upon hire.
• Board of Pharmacy Specialty certification (BPS) or other approved certifications such as Certified Specialty Pharmacist (CSP) or AAHIVP are preferred upon hire.

Knowledge, Skills, and Abilities

• Strong verbal and written communication skills.
• Basic computer skills are preferred.

This position offers a competitive salary range of USD $120,702.00 - USD $238,222.00 per year.
