As a key member of our nursing team, you will play a vital role in providing high-quality care to our residents. About the Role:

As an LPN at Axiom Gardens of Mount Vernon, you will be responsible for:

Providing direct patient care, including administering medications, wound care, and vital sign monitoring
Assisting with daily living activities, such as bathing, dressing, and grooming
Collaborating with our interdisciplinary team to develop and implement individualized care plans
Maintaining accurate and detailed records of patient care and progress
Ensuring a safe and clean environment for residents and staff

Requirements:

Current LPN license in the state of Illinois
High school diploma or equivalent required; associate's or bachelor's degree in nursing or related field preferred
