Physical Therapist Opportunity

Elite Therapy Providers is a therapist owned company looking for both part and full time recent graduates to perform contract home health PT in Orange, Los Angeles, Riverside, and San Bernardino Counties. We provide comprehensive training for those without home health experience to maximize patient care time. Electronic documentation is simple and can be completed during the visit. We have a high volume of referrals to keep therapists earning.

Essential Functions:

• Develops a plan of care in collaboration with the patient, family/caregiver, and physician including frequency and duration of therapy visits, treatments, appropriate goals, request for other disciplines as needed, and discharge planning.
• Performs and documents PT interventions including patient education according to Plan of Care
• Completes all assessments/evaluations, including OASIS if necessary, at specific time-points as defined by regulation and agency policy, accurately and timely.

Required Licenses/Certifications:

• California Physical Therapy licensure
• Basic Life Support certification
• Current California driver's license
• Proof of auto liability insurance
• Proof of professional liability insurance

Compensation: $80.00 - $125.00 per hour
