Cahaba Medical Care Foundation is seeking a Counselor to provide behavioral health services at the SBHC Central Office in Centreville, AL. The role includes individual and group counseling for children, adolescents, adults, and families within a primary care setting. You will collaborate with the primary care team to develop care plans, assess crises, and support self-management of chronic conditions.
