Responsibilities:

• Provide direct patient care, including assistance with daily living activities, vital signs monitoring, and mobility exercises

• Develop and maintain effective relationships with residents, families, and healthcare teams

• Collaborate with licensed nurses to develop and implement individualized care plans

• Maintain accurate and detailed records of resident care

• Participate in quality improvement initiatives and contribute to a positive and respectful work environment
