You Will Enjoy: 401(k) with company match Resident sponsored educational scholarships Employee assistance program Free electric vehicle charging stations Daily Pay - Work today, get paid today An exceptional work environment that is both engaging and fun Here are a few of the responsibilities of a Community Social Worker: Support coordination of resident care and services across the community's continuum of care Serve as a liaison between residents, families, physicians, and community staff to ensure clear communication and coordination of care Maintain accurate, current, and confidential social service documentation and resident records Assist residents in accessing internal and external resources Support Social Services in ensuring advanced directives are completed, updated, and properly maintained in resident records Support resident wellbeing by assisting with wellness initiatives Coordinate short-term and long-term stays within higher levels of care Here are a few of the qualifications we need you to have Degree in Social Work, Gerontology, or a related field Licensure as a Social Worker in Connecticut is preferred 2-3 years of experience in long-term care, transitional care, memory care, or equivalent healthcare settings preferred Proficiency in technology, including Microsoft Office Suite Strong interpersonal and communication skills All new hires must be able to pass our preemployment screening process, which includes a fitness for duty physical, criminal background check, drug test and employment references as required by a licensed residential care facility.
