Actify Home Care has active LPN home health cases ready for assignment across our expanding territory. We can fast-track your credentialing and get you working quickly!
LPN Compensation (1099):
Hi-Tech / Complex Care Visits: $35.00
Routine Follow-up Visits: $30.00
Territory: South Palm Beach County : Boca Raton, Delray Beach, Boynton Beach, Lantana, and surrounding areas.

Actify Home Care is clinician-owned by a Physical Therapist and clinically led by a veteran Director of Nursing—each with over 20 years of home health experience. What You’ll Do as an LPN:
Patient Care: Provide high-quality, one-on-one therapy to geriatric patients in their homes.
Care Plans: Implement and follow treatment plans established by the supervising RN.
Timely Documentation: Chart patient progress and treatment responses accurately and efficiently.
RN Collaboration : Coordinate and communicate patient status regularly with the supervising RN.
What You Need:
License: Active Florida Registered Licensed Practical Nurse (LPN) License
Certification: Current CPR/BLS Certification.
Transportation: Valid Driver’s License and reliable transportation.
Software: Kinnser/WellSky experience is a major plus!
