Registered Nurse (RN) Case Manager

We are seeking an experienced Registered Nurse (RN) Case Manager to join our team in an Inpatient Rehabilitation Facility (IRF) setting. This role requires a dedicated nursing professional to manage patient care and coordinate discharge planning effectively.

Responsibilities

• Complete initial discharge planning assessments within two days of patient admission.
• Participate in the Initial Interdisciplinary Team Meeting on or before the fourth day of the patient's stay, followed by weekly meetings thereafter.
• Be responsible for all discharge planning needs, including arranging Home Health, Outpatient Therapy, Durable Medical Equipment (DME), and Dialysis chair setup/confirmation.
• Conduct post-discharge follow-ups within 72 hours and again at 30 days post-discharge.
• Oversee continued stay payer reviews and ensure the re-issuance of the Important Message from Medicare as required.

Qualifications

• Must possess a valid Texas RN license.
• Basic Life Support (BLS) certification is required.
• Minimum of 2 years of experience in acute care or inpatient rehabilitation case management or utilization review.
• Familiarity with WellSky electronic medical record (EMR) system is preferred.
• Experience with insurance payer portals, specifically Availity and Home & Community, for continued stay authorization reviews.

Work Schedule

This position requires availability for a Monday to Friday schedule from 8 AM to 5 PM, with flexibility for weekend and after-hour admissions, working between multiple locations in the region.

Additional Information

The average patient length of stay is approximately 11.2 days. Quick turnaround and effective discharge planning are critical to the success of this role. An interview is required for all candidates.
