Job Title: Home Health RN Location: Nescopeck, PA, USA Employee Type: Full Time Working Hours Per Week: 40 Job Description Residential Home Health and Hospice (Residential) is looking to add a Home Health Registered Nurse Case Manager to our extraordinary care team. You will lead and collaborate with our care team to provide management and delivery of patient care plans for patients in Nescopeck, PA, and surrounding areas. Responsibilities Collaborate with interdisciplinary team to manage and deliver patient care plans. Provide direct patient care and assess clinical needs. Submit accurate charting and meet critical deadlines. Self‑manage time and schedule. Use tablets, mobile phones, and EMR software to document and manage care. Qualifications RN license in the state of employment. Graduate from an approved school of practical nursing. Minimum one year of RN experience and clinical competence to deliver quality patient care. Current driver’s license with ability to spend ~20% of day driving to/from patient locations. Demonstrated experience with tablets, mobile phones, and EMR software.
