Physical Therapist - Home Health

Job Overview

The Physical Therapist will provide effective and compassionate care in a home health setting. This role involves managing patient assessments, ongoing treatment plans, and ensuring quality care is delivered in compliance with regulations and best practices.

Key Responsibilities

• Conduct start of care assessments and evaluations.
• Perform recertifications and resumption of care for ongoing patients.
• Manage a typical caseload of 5-6 patient visits per day, which may vary based on admissions or additional assessments.
• Collaborate with clinical coordinators and directors to address patient needs and questions while in the field.
• Provide support in rare Hospice situations as needed for safety evaluations or teaching visits.

Work Environment

This position operates under a Monday through Friday schedule, from 8 AM to 5 PM, with no weekend shifts required. Holidays and weekends are off.

Orientation and Training

New hires will receive a brief orientation period, working alongside a preceptor for 1-2 weeks to ensure a smooth transition into the role.

Requirements

• Current BLS certification.
• Active Physical Therapy License in North Dakota.
• Must have a valid Driver's License and Auto Insurance.
• Home health experience and OASIS training are preferred.

Professional Appearance

Uniforms must adhere to the following color guidelines: black, navy blue, or grey scrubs.

Additional Information

This position does not have on-call requirements. The physical therapist will benefit from support through experienced colleagues in related agencies working the same hours.
