Registered Nurse - Home Health

Job Description

Job Summary:
Responsible for providing comprehensive in-home nursing care to patients within the home health setting. The RN Case Manager collaborates with interdisciplinary teams to develop and implement personalized care plans, ensuring optimal patient outcomes.

Responsibilities:

• Develop and review individualized care plans in collaboration with physicians, patients, and families.
• Coordinate patient care activities through effective communication with team members and participation in case conferences.
• Assess patient progress regularly, identifying and addressing clinical, functional, and emotional health issues.
• Document evaluations, treatment goals, and progress reports accurately and in a timely manner.
• Educate patients and caregivers on self-care techniques, medication management, and health maintenance.
• Communicate significant changes in patient status to physicians and team members to ensure continuity of care.

Qualifications:

• Hold a valid, unrestricted Registered Nurse license in Vermont and New Hampshire or a current compact license.
• Graduate from an accredited nursing program.
• Possess at least 2 years of professional experience in home health nursing.
• Demonstrate strong assessment and communication skills.
• Ability to work independently and collaboratively within a team environment.
