RN Case Manager: Care Coordination & Discharge
The Christ Hospital
The Christ Hospital is seeking a dedicated Case Manager to oversee utilization review, discharge planning, and care coordination for acutely ill patients in Cincinnati, OH. You will collaborate with physicians, nurses, social workers, and external payers to ensure appropriate level of care, timely treatment, and patient education throughout the hospitalization and discharge process.
