Clinical Pharmacist

Job Requisition: Clinical Pharmacist As a Clinical Pharmacist, you will play a vital role in optimizing medication therapy for our patients. You will collaborate closely with physicians, nursing staff, and other healthcare professionals to ensure safe and effective medication use. Key Responsibilities Conduct comprehensive medication reviews and assessments for patients. Collaborate with healthcare teams to develop individualized medication plans. Educate patients and healthcare staff on medication therapy and potential side effects. Monitor patient outcomes and adjust medication regimens as necessary. Participate in interdisciplinary rounds and contribute to patient care discussions. Ensure compliance with pharmacy regulations and standards of practice. Qualifications Doctor of Pharmacy (Pharm.D.) degree from an accredited institution. Current pharmacy licensure in the relevant state. Previous clinical pharmacy experience preferred.
