Registered Nurse - Home Health

Job Description

Job Summary:
Provides comprehensive home health nursing care to patients, ensuring adherence to physician orders, agency policies, and standards of clinical practice. Responsible for coordinating patient care, supervising home health aides and licensed nurses, and maintaining accurate documentation.

Responsibilities:

• Coordinate and deliver skilled nursing care to patients in their homes based on physician orders and patient needs.
• Supervise and evaluate the performance of Home Health Aides, LPNs, and VNs to ensure quality care delivery.
• Document all patient interactions, treatments, and assessments accurately within the required timeframes using HCHB EMR system.
• Travel to patient homes within designated territory, managing a case load of 5-8 patients daily, and ensuring timely visits.
• Participate in periodic office meetings and on-call rotations, providing support and coverage as needed.
• Maintain compliance with licensing, certification, and reimbursement requirements, including OASIS proficiency for RNs.

Qualifications:

• Valid RN license in the state of practice, in hand at time of application.
• Minimum of 1 year of adult home health or hospice experience within the last year.
• Proficiency in HomeCare HomeBase (HCHB) EMR system within the past 6 months.
• Must have a current driver's license, reliable vehicle, and agency-required liability insurance.
• Ability to travel up to 75 miles from the assigned care center and work independently in a home setting.
• Strong clinical assessment and communication skills.
