Private Duty Clinical Team Manager (Home Health)

Full-Time | Mishawaka, IN / $5,000 Sign-On Bonus

Your Role

? Oversee patient care services and clinical staff

? Review and approve plans of care and physician orders

? Ensure timely start of care and regulatory compliance

? Conduct OASIS reviews and support quality improvement

? Coach and mentor field clinicians

? Partner with the Branch Director on performance metrics

? Support hiring and team growth

What You Bring

? Associate degree in nursing

? 2 plus years of RN experience in Home Health or Hospice preferred

? Supervisory experience preferred

? Active Indiana RN license

? Experience with Homecare Homebase and Microsoft Office

? Ability to travel within the coverage area

? Sit, stand, bend, lift, and move intermittently and be able to lift 50 to 100 lbs
