Hospice RN Case Manager — Impactful, On-Site Role

Westminster-Canterbury-On-Chesapeake-Bay in Virginia Beach invites applications for a Registered Nurse (RN) Case Manager to join our dedicated hospice care team. The ideal candidate will bring a compassionate approach to nursing and a commitment to providing exceptional care to our patients and their families. This is a full-time, on-site position that requires strong communication skills and the ability to work collaboratively within an interdisciplinary team. Duties encompass comprehensive patient assessments, care plan implementation, and patient education.
