Orthopedic At-Home Physical Therapist – Per Diem (Middlesex/Mercer County) Orthopedic At-Home Physical Therapist – Per Diem (Middlesex/Mercer County) - JAG Physical Therapy seeks a skilled Physical Therapist specializing in orthopedic rehabilitation in a home setting for patients recovering from total hip and knee replacement surgeries and other orthopedic‑related conditions. The role involves evaluating and treating patients in the home setting, with the goal of transitioning them to JAG outpatient orthopedic locations for continuity of care. Responsibilities Educate patient on safe bed mobility, transfers and ambulation in consideration of post‑operative precautions. Transfer, ambulate and perform stair negotiation with patients to determine they are safe for discharge to home. Collect and record patient care data following the format and guidelines of the patient care service, regulatory agencies and third‑party payers. Perform initial evaluation, daily notes, reassessments/progress reports, discharge plans and communicate with members of the team. Complete daily billing and follow department guidelines for productivity. Prepare for evaluation in accordance with department policies, procedures and standards of practice, including obtaining patient history, exhibiting knowledge of patient diagnoses and researching unfamiliar diagnoses. Select and administer tests and measures. Synthesize evaluation results to formulate the diagnosis, goals, prognosis and plan of care. Establish treatment plan based on evaluation results, goals and best practice; collaborate with interdisciplinary team, patients and patients’ support system. Assess and re‑assess need for modifications to plan of care and goals. Provide education/training to patient and/or patient support system. Functional training: ADLs/IADLs, therapeutic activities (e.g., transfers, bed mobility), rehabilitation equipment prescription and training, and community reintegration. Responsible for treatment, plan of care, exercise prescription for patient caseload. Provide interventions that are safe, effective and compliant with JAG standards. Provide data and note writing using Prompt EMR for all treatments provided. Attend annual JAG Billing and Coding seminar. Communicate with Manager/Patient Access team for scheduling evaluations once demographics are received. Qualifications Bachelor’s or Master’s Degree required (Doctorate preferred). Licensed Physical Therapist in the state practicing in (NY or NJ). Completed state‑regulated CEU requirement in appropriate state (NY, NJ). Valid CPR License. Experience/knowledge with an EMR software preferred. Excellent oral and written communication. Strong clinical decision‑making skills. Excellent work ethic and dedication to patient success. Basic computer skills. Works as a team member and independently with minimal supervision. Maintains a positive, professional demeanor at all times.
