Nurse Practitioner Or Physician Assistant Opportunity In Bronx, NYWe are seeking a quality-driven physician to join our post-acute care team in the Riverdale neighborhood of The Bronx, New York. This is a full-time opportunity (5 days/week) rounding in skilled nursing facilities, with excellent support and training, autonomy, and earning potential.Key ResponsibilitiesPerform comprehensive patient assessments, including medical histories and physical examsDevelop, implement, and adjust individualized care plans based on patient needsMonitor progress and coordinate care to optimize outcomesCollaborate closely with physicians, nursing staff, and other healthcare professionals in a team approachEducate patients and families on health management and preventive strategiesMaintain accurate, timely medical records in compliance with regulationsQualificationsCurrent NP/PA license (State of New York) and DEAExperience in post-acute, acute, ED, or clinic settings preferred, but not requiredStrong clinical foundation and knowledge of healthcare regulatory standardsExcellent communication and interpersonal skillsCollaborative, team-oriented approach with a positive outlook
