Registered Nurse - Behavioral Health -HUDVASH

Preferred Experience

Minimum 1 year Behavioral Health nursing preferred. Community Care Case Management strongly preferred

Assigned Duties

The Housing And Urban Development - Veterans Affairs Supportive Housing (HUD-VASH) Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches in providing complex patient management.

  • Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings.
  • Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.
  • Coordinates care with multiple providers across all levels and sites of care.
  • The HUD-VASH RN develops a plan of care using information from the patient, family, and provider.
  • Bridges the gap between discharge and primary care follow-up. Identifies high-risk patient population and transitions to appropriate discipline (i.e., Primary Care, Home Based Primary Care, Hospice/Palliative Care etc.).
  • Responsible for intensive monitoring, follow-up, and clinical management of high-risk patients. Collaborates with interdisciplinary team members to develop and implement the Veteran Housing Stability Plan.
  • Contributes to the team-based assessment of the ability of individual Veterans to live independently in the community, as part of eligibility determination for Veterans with complex medical and psychological risk factors.
  • Provide recovery-oriented and Housing First services, with the goal of establishing the Veteran independently in the community, at the Veteran's highest level of functioning.
  • The HUD-VASH RN collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines.
  • Demonstrates performance and leadership that is broad enough to improve the care for a group of patients. Provides peers with formal or informal constructive feedback for improvement.
  • Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals.
  • Evaluates HUD-VASH outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Responsible for the documented outcomes at the program or service level.
  • Program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility.
Job Benefits

Pay: Competitive salary, regular salary increases, potential for performance awards

Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA

Licensure: 1 full and unrestricted license from any US State or territory

Work Schedule: Monday thru Friday; 8:00 am to 4:30 pm

Telework: Ad-hoc; based upon the services needs.

Relocation/Recruitment Incentives: Not Authorized

Permanent Change of Station (PCS): Not Authorized. This position potentially requires flexibility in schedule and assignments. When appropriate, Co Coordinators will be expected to assume the responsibility of a Care Coordination and Integrated Case Management (CCICM) Lead Coordinator serving as the primary point of contact for Veterans who require moderate to complex care coordination, oversee, and facilitate integrated care across VA service lines, healthcare settings, and treatment episodes ensuring seamless and comprehensive care is provided.
