Member Acquisition Specialist Santa Clara County, CA (Field-Based) Compensation: $26-$28/hour (Full-Time) About Us Zócalo Health is a tech-enabled, community-oriented primary care organization serving people who have historically been underserved by the one-size-fits-all healthcare system. We partner with health plans, providers, and community organizations to deliver culturally competent primary care, behavioral health, and social care. We combine local, community-based teams with virtual care and modern technology to deliver coordinated, whole-person care where members live and receive support Founded in 2021, Zócalo Health is backed by leading healthcare and mission-aligned investors and is scaling rapidly across states and populations. We are building a durable care platform designed to perform in constrained healthcare environments and to lead the shift toward accountable, value-based care. Role Description The Member Acquisition Specialist is a field-based role responsible for converting community outreach and referral opportunities into enrolled Zócalo Health members. This role focuses on direct member engagement, enrollment conversations, same-day registration, and follow-through to care connection. Member Acquisition Specialists are deployed strategically across community partner sites, referral locations, and Zócalo-hosted community events to maximize enrollment outcomes and member conversion. Success in this role is measured by enrollments, registration quality, conversion rates, follow-up execution, and the ability to build trust that results in members connecting to care. This is a highly accountable, performance-driven role for individuals who are comfortable initiating conversations, guiding individuals through enrollment processes, overcoming barriers to care, and working independently in community settings. This position reports to the Market Growth Manager. The Member Acquisition Specialist will contribute in the following ways: Enrollment & Conversion
- Conduct in-person outreach and enrollment conversations with eligible community members
- Drive same-day registrations and maximize conversion opportunities in the field
- Guide individuals through registration, intake, and enrollment processes
- Own weekly enrollment, registration, and conversion goals
- Maintain disciplined follow-up to move referrals and leads into enrolled members
- Identify and escalate barriers preventing enrollment or engagement
- Support member retention and re-engagement efforts when appropriate
- Execute enrollment activities at assigned partner sites, community locations, and outreach events
- Represent Zócalo Health professionally within community settings
- Build trust quickly and effectively with community members
- Support execution of Zócalo-hosted events designed to generate enrollment opportunities
- Maintain flexibility to work in field-based environments, including evenings and weekends as needed
- Perform other responsibilities and tasks as requested by your manager. Zócalo Health is a rapidly scaling startup, and with that comes the expectation to occasionally perform additional tasks to help move the business forward.
- Accurately complete registrations, documentation, and enrollment workflows across operational systems
- Maintain timely and accurate activity tracking within Athena and other required platforms
- Follow established operational workflows, SOPs, compliance expectations, and documentation standards
- Communicate operational issues, field insights, and workflow barriers in real time
- Work closely with Market Growth Managers to support community-based enrollment strategy
- Contribute to a high-performance, enrollment-focused team culture
- Maintain strong communication regarding field activity, conversion challenges, and operational needs
- Consistently meets or exceeds enrollment and registration goals
- Demonstrates strong same-day conversion performance
- Maintains disciplined follow-up and pipeline management
- Converts community engagement opportunities into enrolled members
- Operates independently and executes effectively in field environments
- Maintains accurate and compliant documentation across systems
- Builds trust quickly and translates relationships into measurable enrollment outcomes
- Demonstrates urgency, accountability, and strong follow-through

- 1-3+ years of experience in community outreach, healthcare enrollment, public health, sales, canvassing, customer engagement, or field-based roles
- Comfortable initiating conversations and engaging community members in public settings
- Experience working toward performance goals, outreach targets, or enrollment metrics preferred
- Strong communication and relationship-building skills
- Organized, reliable, and comfortable working independently in field environments
- Comfortable using technology and documentation systems
- Bilingual English-Spanish required
- Reliable transportation and willingness to travel locally
- Flexible schedule, including evenings and weekends as needed.
- A strong communicator who knows how to build trust quickly in diverse communities.
- Comfortable with field-based outreach and performance-driven environments.
- Experience with Medi-Cal, ECM, Community Supports, or community-based healthcare programs
- Experience with Athena health or comparable systems
- Experience with field outreach, enrollment support, canvassing, or community activation work
- Familiarity with local community resources and populations served
